What to know before you visit
A little preparation makes your appointment go further.
Meet and Greet
If you’re a new patient, welcome. We’re excited to get to know you. Most physicians will usually have a longer “Meet and Greet” appointment during your first visit, where they get to know you and your health history. They may deal with active issues at this appointment, or ask you to book again soon to discuss current concerns.
What do I need to bring?
Bring all your current prescription medications and your up-to-date Medicare card. If you’re a new patient, you may also fill out and bring a New Patient Questionnaire, or complete one in our waiting room on arrival.
It is also helpful if we know:
- Family history: whether your first-degree relatives (parents, siblings, children) have had heart disease, diabetes, strokes, or cancers, and at what age they were diagnosed.
- Medications: an up-to-date list, or your medications in their packages.
- Surgeries, hospitalizations, or illnesses: the year, and for surgeries which surgeon operated. (Example: diagnosed with hypertension in 2010.)
- Screening tests: when your last cervical screening (Pap or HPV test), mammogram, diabetes, cholesterol, bone density, and colon cancer (stool testing) screens were.
- Immunizations: when your last tetanus, flu, or pneumonia shot was.
